Select the child you’d like to interact as.
To view teams and schools that your child belongs to, navigate to the icon that looks like two people standing behind one another. Once there, look to the top navigation bar – teams will appear on the left and schools will appear on the right. Swipe the screen side to side to move between them.


Select the child you’d like to interact as.
From the teams and schools page, select whether you are searching for a sports team (left) or a school (right). Once on the correct spot, press the blue “+” button in the bottom right corner of the screen. Begin typing in the team or school name. You can also press on the button below the type box that says either “Teams” or “Schools” to apply filters to search criteria.
If the school or team does not appear, it means that the coach or teacher has not created the team or school/classroom yet.


Select the child you’d like to interact as.
Once you add your child to a team, you must also join the roster of each coach/teacher/trainer you’d like to allow to see your child’s name and injury status on their roster. This is set up for your privacy and security because although your child may be on a team, you may not want to share your child’s information with all of the coaching staff. This feature gives you the ability to be in control of sharing your information.
Start by selecting the teams & schools screen and select the team/school you are trying to join. At the bottom of the screen you will see the names of each of the coaches and trainers associated with your team. Be sure to toggle on each of the staff you’d like to share your profile with. Note: coaches can only see limited personal information such as your name, contact details, and injury status. No other health-related data is shared.

From the teams and schools page, click on the team/school that you’d like to leave. Once inside press the red “Leave Team” button. If you’d like to stay associated with the team but would prefer to not share your child’s information with some of the coaching staff or certain teachers, you can toggle them off the “On Roster” button.


Only coaches and teachers can report suspected concussions through the app. This app is meant to be used as a documentation tool for concussions that occur in a school or organized sports setting. As such, any suspected concussions should be brought to the attention of the teacher, coach, or trainer of the team so that they may file an injury report.

From your child’s profile you can upload a profile picture, change account settings (personal information, contact details, email address, or password), view baseline test results, view submitted injury reports. Currently you are unable to send baseline results over the app. But you can take screenshots and send those. You can see current baseline test on the left tab at the top of the screen and you can see test results from previous years by swiping to the right tab labelled “history”.

Select injury reports and then select which report you’d like to see based on the date it was filed. To send the injury report press the share icon in the top right corner of the screen. You’ll then be able to send the report via SMS, email, or print it. The report also contains a referral form for your child’s doctor as well as a medical clearance letter to return back to sport.

Select “Clearances” and then select which clearance letter you’d like to see based on the date it was filed. To send the injury report press the share icon in the top right corner of the screen. You’ll then be able to send the clearance letter via SMS, email, or print it. The report also contains a referral form for your child’s doctor as well as a medical clearance letter to return back to sport. Also, all coaches, teachers, and trainers who have your child on their roster will automatically have access to your clearance letters as soon as they are uploaded.


Clearance letters can only be uploaded by coaches, trainers, or teachers. Again, this app is meant to be used as a tracking and reporting tool for sports clubs and schools. Once your child has received medical clearance to return to full sports, give your clearance letter to their coach, teacher, or trainer to clear you from your injury and upload the letter.
The concussion dashboard is only available to those who have been assessed at a Complete Concussion Management (CCMI) Clinic. If your child is going to a non-CCMI certified clinic, our system has no way of knowing what stage of recovery you are on or what advice to provide you. Go to your profile, select account settings, and scroll down to the “Language” field and select your preferred language.
These features are only available to those patients who are being seen at a Complete Concussion Management Clinic for their injury treatment and rehabilitation. If your child is being seen by one of these clinics, you should have a little red coloured CCMI logo in the middle of the bottom navigation bar. That button takes you to your child’s concussion dashboard which allows you to view all the information necessary to optimize your recovery.
Click on the 3 vertical dots on the far-right side of the bottom navigation bar. This will bring you to the Resources page. From here select “Find-a-Clinic”
